Service Coordination / Case Management

The goal of AUCP’s Service Coordination / Case Management service is to help people locate community resources that will enhance and enrich their quality of life.

Services Include:

  • Finding Community Resources
  • Establishing or managing services
  • In-home assessment for quality services
  • Maintain communication between the participant and services received in the home
  • Funding for this program is provided by the PA Department of Public Welfare.
