Attendant Care Waiver / ACT 150 Program
The goal of the Attendant Care Waiver / ACT 150 Program is to help achieve one or more of the following goals to improve quality of life:
- Live in the least restrictive environment as independently as possible.
- Remain in their own home and prevent unnecessary institutionalization
- Receive in-home personal care
- Receive Service Coordination/ Case Management Services
Eligibility Requirements:
- Age 18 through 59 (over age 59 conditional)
- Meet the required level of care specified in the Pennsylvania Code (See APPENDIX E, SKILLED NURSING CARE, Section II(a)).
- Those who do not meet these requirements MAY qualify for the Act 150 program
- Requires assistance with activities of daily living due to a medically determined physical impairment, which can be expected to last for a continuous period of no less than 12 months
- Mentally alert and capable of selecting, supervising, and if needed, firing an attendant, and managing their own financial and legal affairs
- Be found in need of a basic service
Services Provided:
- Community Transition Services (available only through Medicaid Home and Community Based Waiver Services)
- Financial Management Services
- Participant-Directed Community Supports
- Participant-Directed Goods and Services
- Personal Assistance Services including assist with transfers, bathing, dressing, meal prep, and light ancillary services
- Personal Emergency Response System (PES)
- Service Coordination
- Service Coordination / Case Management
