Community HealthChoices (CHC) is Pennsylvania’s mandatory managed care program designed to support:

  • Dually eligible individuals (those enrolled in both Medicare and Medicaid)
  • Adults with physical disabilities who qualify for long-term services and supports (LTSS)

The goal of CHC is to empower participants to live with dignity and choice, while receiving high-quality services in their own communities. The program offers participants greater control over their health and long-term care, so they can live more independently, work if they choose, and experience a better quality of life.

CHC is about more than healthcare—it’s about giving people control over their lives, improving care coordination, and supporting individuals in the environments they choose to call home.

Program Objectives

  • Provide high-quality services that improve health outcomes and quality of life

Long-Term Services & Supports (LTSS)

CHC participants who qualify for LTSS can receive essential help with daily activities, including:

  • Bathing
  • Dressing
  • Meal preparation
  • Medication management
  • Transportation coordination
  • Mobility support

Community Health Choices (CHC)

Community Health Choices (CHC) is Pennsylvania’s mandatory managed care program for dually eligible individuals and individuals with physical disabilities — serving more people in communities, giving them the opportunity to work, spend more time with their families, and experience an overall better quality of life. CHC will improve services for hundreds of thousands of Pennsylvanians. CHC has been developed to:

  1. Enhance access to and improve coordination of medical care.
  2. Create a person-driven, long-term support system in which people have choice, control, and access to a full array of quality services that provide independence, health, and quality of life.

Long-term services and supports (LTSS) help eligible individuals to perform daily activities in their home such as bathing, dressing, preparing meals, and administering medications.
