Attendant Care Waiver / ACT 150 Program

Supporting Individuals in Living Independently at Home

The Attendant Care Waiver / ACT 150 Program assists individuals with physical disabilities in achieving greater independence and receiving care in the least restrictive setting possible.

Program Goals

  • Enable independent living in the least restrictive environment
  • Remain in one’s own home, preventing unnecessary institutionalization
  • Receive personal care and assistance with daily activities
  • Access Service Coordination and Case Management support

Eligibility Requirements

  • Age 18–59 (conditional eligibility for individuals over 59)
  • Meet the required level of care per Pennsylvania Code (APPENDIX E, Skilled Nursing Care, Section II(a))
  • Have a medically determined physical impairment expected to last at least 12 continuous months, requiring assistance with activities of daily living
  • Be mentally alert and capable of selecting, supervising, and if necessary, firing an attendant, and managing financial/legal affairs
  • Must be in need of at least one basic service

Community Transition Services
Waiver-only assistance for transitioning out of institutional care
Financial Management Services
Assistance with managing program funds and employer responsibilities
Participant-Directed Community Supports
Self-managed support services tailored to individual needs
Participant-Directed Goods & Services
Purchasing approved items to meet support needs not otherwise covered
Personal Assistance Services
Assistance with bathing, dressing, meal prep, light cleaning, transfers, and mobility
Personal Emergency Response System (PERS)
A medical alert system to ensure emergency access and response
Service Coordination
Help navigating and linking to services, supports, and resources
Case Management
Ongoing oversight and assistance from a dedicated service team
