

Attendant Care Waiver / ACT 150 Program
Supporting Individuals in Living Independently at Home
The Attendant Care Waiver / ACT 150 Program assists individuals with physical disabilities in achieving greater independence and receiving care in the least restrictive setting possible.
Program Goals
- Enable independent living in the least restrictive environment
- Remain in one’s own home, preventing unnecessary institutionalization
- Receive personal care and assistance with daily activities
- Access Service Coordination and Case Management support
Eligibility Requirements
- Age 18–59 (conditional eligibility for individuals over 59)
- Meet the required level of care per Pennsylvania Code (APPENDIX E, Skilled Nursing Care, Section II(a))
- Have a medically determined physical impairment expected to last at least 12 continuous months, requiring assistance with activities of daily living
- Be mentally alert and capable of selecting, supervising, and if necessary, firing an attendant, and managing financial/legal affairs
- Must be in need of at least one basic service
