

Service Coordination / Case Management
The goal of AUCP’s Service Coordination and Case Management program is to help individuals locate and connect with community-based resources that enhance and enrich their quality of life. Our dedicated coordinators work hand-in-hand with participants to provide guidance, advocacy, and access to vital services that support independent living.
Our Services Include:
- Locating and connecting participants to local community resources
- Establishing and managing personalized service plans
- Conducting in-home assessments to ensure quality and safety of care
- Maintaining consistent communication between the participant and care providers
Funding for this program is provided by the Pennsylvania Department of Public Welfare.
