Nursing Home Transition

The goal of the Nursing Home Transition Program is to assist adult residents of long-term care facilities, who are 18 years of age and older, move out of the nursing home and into their own independent community. Nursing Home Transition starts with the collaboration of the resident, the staff of the long-term facility, the resident advocates, the family members, and other service providers.

Services Include

  • Helping nursing home residents locate appropriate housing and deciding where they want to live
  • Coordinating the professional services required to effect the move from nursing home to independent living in the community
  • Establishing the household and connecting the individual with such needed services as medical, home health-care, or transportation, among others
