Independence Waiver (For Physical Disabilities)

The Independence Waiver is a Home and Community Based program that provides services to persons with physical disabilities to allow them to live in the community and remain as independent as possible.

Eligibility Requirements:

  • Age 18 through 60
  • Persons with physical disabilities
  • Meet the required level of care specified in the Pennsylvania Code (See APPENDIX E, SKILLED NURSING CARE, Section II(a)).
  • Have a disability that substantially limits at least three of the following functions:
    • Self-care
    • Receptive and expressive language
    • Learning
    • Mobility
    • Self-direction
    • Capacity for independent living
    • Economic self-sufficiency
    • Disability is expected to continue indefinitely

  • Adult Daily Living Service
  • Assistive technology/specialized medical equipment and supplies
  • Accessibility Adaptations, Equipment, Technology and Medical Supplies
  • Community Integration
  • Community Transition Services
  • Environmental Adaptations/ Home Modifications
  • Financial Management Services
  • Home Health
  • Non-Medical Transportation
  • Personal Assistance Services including assist with transfers, bathing, dressing, meal prep, and light ancillary services
  • Personal Emergency Response System (PERS)
  • Respite
  • Service Coordination / Case Management
  • Supported Employment
  • Therapeutic and Counseling Services (Behavioral, Occupational, Physical, Speech)
